1 FTE role covering practices in the REAL local community network. This is a fixed term contract until 26th May 2020 with a possibility to extend.


You will be working out of the following practices: Coppice and Angmering Medical Centre Fitzalan Medical Group The Park Surgery Westcourt Medical Centre Willow Green Surgery

Job description:

The REAL Social Prescribing pilot is a one-year collaborative project between Arun and Chichester Citizens Advice, Arun Primary Care Group Ltd and the Local Community Networks (LCNs). Arun and Chichester Citizens Advice as a partner of the LCNs will be the employing agency.

Role purpose:

The project will provide a new referral system for GPs and other Healthcare professionals to refer patients with social, emotional or practical needs to a range of non-clinical services. This is based on the belief that the best way to tackle around 70-80% of health issues lies in non-medical solutions. The Community Referrers will provide individual support for patients referred into the service to access a range of community based non-clinical services. Using behaviour change techniques, they will work with individuals to set goals and priorities which will lead to improvements in their health and wellbeing.

  • · Be able to assess where a patient might benefit from advice and support
  • · Be understanding toward patients social difficulties
  • · Flexible with hours
  • · Have experience in career support / care work / charity work or work of a similar nature
  • · Have a driving licence and own transport (due to supporting the different medical centres)
  • · Any counselling or behaviour technique experience is advantageous
